Read this before every appointment. A clean form gets your client approved faster, and gets you paid faster.
This is called clean sheeting. Do not do it. Not to help the client. Not to get the policy issued. Never. The insurance company looks up their old records, their medicines, and their driving. They will find it. If they find it later, they cancel the policy. Then the family gets back the money they paid in instead of the full payout. You can also lose your license.
The client signs. The client puts the date. The client fixes their own mistakes. Never sign their name for them. Never send in a form that is already signed.
Something wrong? The client fixes it and puts their initials next to it. Or you start a new form.
Never leave a box empty. If it does not apply to the client, type N/A. Then nobody has to wonder if you skipped it.
Say this to the client first:
Those few sentences stop most of the problems before they start.
Read every question the way it is written. Do not put it in your own words. Do not decide for the client what matters.
If the client pauses, or asks "does that even count?" — the answer is yes. Write it down. Let the insurance company decide what matters.
Could our coordinator finish this form without calling you once?If yes, you did it right.
